I authorize the York North Basketball Association (YNBA) to collect and use personal information about myself/my child/ward, including name, address, email, telephone number, cell phone number, sex, age, date of birth, proof of age and any other additional information required by the Club for its own use for the following purposes:
To constitute basketball teams according to age, gender, physical and skill ‘criteria’
To provide to volunteer staff for coaching and administrative purposes
To register players for informational and insurance purposes with the Ontario Basketball Association (OBA) and various leagues/tournaments/competitions in which YNBA players may participate
To constitute and maintain a list of club members, for contact by the YNBA from time to time for the distribution of information by telephone, email and mail
Information collected will be stored for the shorter of up to ten years, or specific request for deletion by a registrant, and available for correction or review
Registrant pictures to be published on the website and newspapers or other promotional materials
The YNBA will not sell or provide your information to third parties. We may pass third party information on to you if we feel it is of interest to our members.
